Dennoch ist der verantwortungsvolle Umgang mit Antibiotika bei allen Tierarten angezeigt. Der Einsatz der Antibiotika unter Zuhilfenahme von Resistenzmonitoring und auf der Grundlage von Antibiogrammen ist damit immer der Idealfall. In Kurzform: Velagliflozin ist ein wunderbares Medikament f\u00fcr diabetische Katzen, welches eine orale Diabetes-Therapie erm\u00f6glicht. Aber die Katzen m\u00fcssen noch ausreichend eigene Insulinreserven besitzen. Ansonsten wird es gef\u00e4hrlich. Ohne diese Insulinreserven kommt es zur Bildung von Ketonen. Das gr\u00f6\u00dfte Risiko ist dabei die nicht erkannte Ketoazidose mit m\u00f6glicher Todesfolge. Diabetiker unter Velagliflozin haben fast immer einen normalen Blutzucker, k\u00f6nnen aber trotzdem in die Ketoazidose abrutschen. Eine Situation, die f\u00fcr uns Tier\u00e4rzte neu ist. Denn sonst kennen wir die diabetische Ketoazidose nur in Zusammenhang mit einer Hyperglyk\u00e4mie. Somit: Bei Einsatz von SGLT-2 Inhibitoren z\u00e4hlt die Keton-Messung, nicht die der Glukose! Die Kontrolle der Ketone sollte insbesondere in der ersten Woche sehr engmaschig (optimal t\u00e4glich) erfolgen. Oft wird hier der Harnstreifentest angeraten. Aber: Denken Sie daran, dass der Harnstreifenstest bei Katzen oft erst dann Ketone anzeigt, wenn die Ketoazidose bereits vorhanden ist. Das ist im Zweifelsfall zu sp\u00e4t. Daher raten viele Endokrinologen zur Messung von \u03b2-Hydroxybutyrat im Blut. Mittlerweile werden auch Vorschl\u00e4ge f\u00fcr m\u00f6gliche Cut-Offs gemacht. So wird eine Konzentration von \u03b2-Hydroxybutyrat im Blut von &gt; 2,4 mmol\/l oft als kritisch angesehen und ein Wechsel zu einer Insulintherapie wird angeraten.
