Als \u00dcbertr\u00e4ger sind Zecken des <em>I. ricinus\/I. persulcatus<\/em>-Komplex, in Deutschland <em>I. ricinus <\/em>(Gemeiner Holzbock), beschrieben, mit h\u00f6chster Aktivit\u00e4t im Fr\u00fchling und Fr\u00fchsommer. Die Verbreitung der Erkrankung korreliert mit der Verbreitung der Zecken als Vektoren. Vor allem in Nord-, aber auch Zentraleuropa, sind die h\u00f6chsten Pr\u00e4valenzen bei Hunden, Katzen und Pferden beschrieben. Neben der \u00dcbertragung durch Zecken kann der Erreger auch durch Bluttransfusionen \u00fcbertragen werden. Durch den Klimawandel sind Infektionen mit <em>A. phagocytophilum <\/em>mittlerweile ganzj\u00e4hrig m\u00f6glich, mit Hochphasen in den Sommermonaten.<\/p>\n<p><strong>Zum klinischen Bild <\/strong>\u2013 bei Tieren und Menschen sind als klinische Symptome Fieber, Lethargie und Inappetenz beschrieben. Seltener sind Lahmheiten und\/oder gastrointestinale Symptome bekannt. Diese Symptome sind sehr unspezifisch und weisen nicht klar auf eine bestimmte Erkrankung hin, so dass weitere Testverfahren zur Diagnose erforderlich sind. Bei der granulozyt\u00e4ren Anaplasmose handelt es sich um eine Akuterkrankung, bei der meist ein pl\u00f6tzliches Auftreten von Symptomen beobachtet wird.<\/p>\n<p><strong>Zur Diagnostik <\/strong>\u2013 eine schnelle und kosteng\u00fcnstige M\u00f6glichkeit ist die mikroskopische Untersuchung eines peripheren Blutausstrichs. In den neutrophilen Granulozyten, selten einmal auch in eosinophilen Granulozyten, k\u00f6nnen sogenannte Morulae (Einschlussk\u00f6rperchen) nachgewiesen werden (Abb. 1). Eine hoch sensitive und spezifische Nachweismethode stellt die PCR aus EDTA-Blut oder Synovia dar, die als Goldstandard zur Diagnosestellung gilt (Anaplasma phagocytophilum-PCR 38,00 \u20ac netto Tierarzt). Antik\u00f6rpernachweise aus dem Serum\/Plasma best\u00e4tigen lediglich einen Erregerkontakt in der Vergangenheit, sind jedoch zur Diagnosestellung einer akuten granulozyt\u00e4ren Anaplasmose nicht geeignet.<br \/>\nAntik\u00f6rperspiegel bleiben nach Erregerkontakt oft jahrelang erh\u00f6ht (Anaplasma phagocytophilum-AK 29,00 \u20ac netto Tierarzt).<\/p>\n<p><strong>Zur Therapie und Prognose <\/strong>\u2013 die Prognose ist bei rechtzeitiger Therapie mit einem intrazellul\u00e4r wirksamen Antibiotikum sehr g\u00fcnstig. Bei Hunden (Dosierung 5 mg\/kg KGW BID p. o.) und Katzen (10 mg\/kg KGW SID p. o.) sowie dem Menschen gilt Doxycyclin als Medikament der Wahl. Auch wenn h\u00e4ufig eine sehr schnelle klinische Besserung eintritt, sollte Doxycyclin bei Hunden und Katzen \u00fcber (21\u2013) 28 Tage gegeben werden. Bei Katzen sollten die Tabletten mit Futter\/Wasser verabreicht werden, um \u00d6sophagitis oder \u00d6sophagusl\u00e4sionen zu vermeiden. Bei Pferden sind Oxytetracyclin und\/oder Doxycyclin in verschiedenen Therapieprotokollen beschrieben. Ziel der Therapie ist die Erregerelimination. In Einzelf\u00e4llen traten bei zu fr\u00fchem Absetzen der Antibiose erneut klinische Symptome mit einer positiven <em>A. phagocytophilum<\/em>-PCR auf. Eine Therapiekontrolle mittels PCR wird 5\u20138 Tage nach Abschluss der Therapie empfohlen.<\/p>\n<p><strong>Zur Prophylaxe <\/strong>\u2013 es steht momentan kein Impfstoff gegen <em>A. phagocytophilum <\/em>zur Verf\u00fcgung. Daher beschr\u00e4nkt sich die Prophylaxe auf einen ganzj\u00e4hrigen Schutz gegen Zecken, in Zentraleuropa mit gegen <em>I. ricinus <\/em>zugelassenen Pr\u00e4paraten.<\/p>\n<p><strong>Zur Reinfektion <\/strong>\u2013 Antik\u00f6rper, die im Zuge einer Infektion mit <em>A. phagocytophilum <\/em>gebildet werden, gelten als nicht protektiv.
